Survey

I am posting this survey to make sure that our board of directors hears input from the residents.
Take this survey to be sure that your elected Director knows your preference. Tell your neighbors about it.
In order to provide some level of validity to this (so people don't try to influence the survey by voting more than once) I ask you to provide your name (as shown on your voter registration). This information is optional, however I won't count your vote if you don't use your real information.
Your information will not be shared with anyone. The data will be tallied and presented to the hospital board as a summary.
